Who is Key Medical Care?

Key Medical Care (KMC), founded by our Chief Medical Officer, Dr. Steve Arze, is a vital part of the SilverStone Health family. The KMC team consists of board-certified physicians, nurse practitioners, nurses, clinical assistants, and social workers, all skilled in addressing immediate acute-care and chronic healthcare needs. They provide both in-person and online medical support throughout your healthcare journey, ensuring your needs are met in long-term care facilities, and during transitions to home or new care environments.

If you don’t have a primary care physician (PCP), they can serve as your PCP or act as a bridge to your existing PCP. They assist with securing appointments, obtaining medications, and treating common illnesses like sinus infections, as well as advanced diseases such as COPD. This ensures that you receive the care you deserve every step of the way.

Physician Services provides expert in-person medical care for residents in long-term care and skilled nursing facilities, focusing on your needs during recovery and transitions from hospital to home. To support you after discharge, we offer transitional care through telemedicine, ensuring you have the right medication management and home healthcare support. You can choose from one-time visits or ongoing assistance, and if you need to return to a skilled nursing facility within 30 days, we’ll coordinate that care for you.

  • Anyone can sign up for Specialty Physician Services.
  • Our telemedicine services can be tailored to include one-time visits or ongoing support, depending on the patient’s needs.
  • If patients encounter difficulties at home, we also coordinate care for those eligible to return to skilled nursing facilities within 30 days of discharge.
